View Claims and Explanation of Benefits

Here, you can view paid claims for you and your enrolled dependents.

The Explanation of Benefits (EOB) is created by HMA and contains details of the claim. On the EOB, you will find:

• Patient Name
• Claim Number
• Payment Information

To create a user account, you will need the following:

• A valid e-mail address 
• Your Group Number and Member ID (refer to your ID card)
